2008 Southaven High School Parent Band Contract

 

 

____ (Please initial) I have read the student portion of the handbook and understand the policies that my student is expected to comply with regarding rehearsal procedures, band member guidelines, attendance policy, and care of band property as stated in this handbook.

____ (Please initial) I have read the General Guidelines of the handbook and understand the requirements to participate in any of the Band Classes and the extra-curricular activities offered through the Southaven High School Band Program.

____ (Please initial) I have read the information that is specific to the responsibilities of parents regarding participation and the timely payment of the student’s Fair Share Account (FSA), and I understand that full payment for any activity is required for my student’s continued participation in the extra-curricular activities of the Band Program.
